Loyalty Rewards Plan is our Discount Dental Plan

Benefit Options for Loyalty Rewards Members:

  • 12 months of dental benefits
  • Hygiene or Periodontal Cleanings covered (2-4 times per year, depending on plan)
  • Unlimited X-Rays and Exams
  • No cost for Emergency Exams
  • 20% OFF all treatment
  • No Deductibles
  • No Missing Tooth Clauses
  • No Prior History Exclusions

CV Dental Care’s primary goal is to help you maintain optimal dental health at an affordable price.

Basic fee:

  • Individual- $15 per month or $180 for year.
  • Couples – $27 per month or $324 for year.
  • Family (up to 2 kids)- $37 per month or $444 per month
  • Additional children- $7 per month or $84 per year.

Individuals that need periodontal care will be an additional $15 per month, being they receive 4 cleanings a year.

Ideally patients should pay this fee up front for the year as this is the simplest way, however we do a monthly payment plan.

All  patients need a Doctor or Hygienist to diagnose if there is any periodontal issues before we know what the final plan price will be for the year. To schedule an appointment or answers any questions please fill out the form below.
